Be prepared to give the following information:
To report the passing of a pensioner:
- Deceased members full name and serial number (serial number is for police only)
- Reporting person name and relationship
- Date, place and cause of death
- Date of appointment to Department, if known
- Date and place of birth
- Rank at retirement and last assignment, if known
- Type of pension, i.e., Service or Disability
- Notify the Relief Association of funeral information
- Date, time and location of service, if known
- Survivors request regarding flowers or charitable donation, if known
- Survivors request regarding a Departmental Chaplain, if desired
- If a cemetery plot is needed contact the Los Angeles Firemen's Relief Association
- Notify any Fraternal Organizations if their participation is desired
- The Los Angeles Retired Fire and Police Association will place a commemorative brick/paver at the respective museum
